             If you are in our clinic first time you may be asked to complete our registration form. We will ask you to describe your health history and your symptoms - this knowledge both modifies and dictates diagnosis and proper treatment. Then you may require lab tests – we will contact you with the results of your tests right after we will receive them.

           Please remember try to provide as much information as you can because that can reduce your visit to minimum. We would like to assure you that all information provided is private and confidential.
